The Denver Health Medical Plan, Inc. (DHMP) offers two Medicare Advantage plans--both with Medicare Part D coverage.

Medicare Choice (HMO SNP) is a special needs plan for those eligible for both Medicare and Medicaid benefits.

Medicare Select (HMO) is for all people with Medicare and individuals who qualify for Medicare Savings and Extra Help.

Both programs offer added benefits for members.  To qualify, you must live in Denver County and not have ESRD (End Stage Renal Disease).  For more information, contact Member Services at 303-602-2111.  Our hours of operation are 8:00 a.m. - 8:00 p.m. seven days a week.  Call toll free at 1- 877-956-2111.  TTY users should call 303-602-2129 or toll free at 1- 866-538-5288.
