Thank you for your interest in DHMP Medicare Select© (HMO) . Our plan is offered by DENVER HEALTH MEDICAL PLAN, INC., a Medicare Advantage Prescription Drug Plan (MA-PD). If you have any questions, please call us at 303-602-2111.  Our hours of operation are 8:00 a.m. - 8:00 p.m. seven days a week.  TTY users should call 303-602-2129.  To qualify you must live in Denver County, be eligible for Part A and enrolled in Part B and not have ESRD.

There is a $32.80 monthly plan premium in addition to your $96.40 monthly Medicare Part B premium.

ADDITIONAL BENEFITS:

These benefits are available to Medicare Select members at no additional charge, subject to the limits noted.

  • Dental Services - You are eligible for up to $1,300 for preventive only dental services through Delta Dental.,

  • Hearing Services: 

  • 0% - 20% of the cost for Medicare-covered diagnostic hearing exam

  • 0% of the cost for up to one hearing test every three years.

  • $1,500 for hearing aids every three years.

  • Vision Services: 
  • 0% of the cost for up to one routine eye exam every two years.
  • $150 every two years for eyewear.
  • Transportation - $0 copay for each round trip, up to 8 trips every year, to Plan-approved locations.

  • Health and Wellness:    Medicare Select includes health/wellness education benefits         

    •  Access to our Care Management Program including Health Coaches         

    • Health Club Membership at Denver Park & Recreation Centers

    • Written health education materials including newsletters

    • Nutritional training
